A press conference was organised to mark the completion of two successful years and to announce the new initiatives undertaken in the domain of Infertility treatment. Chief guest – Professor Roy Homburg, Sr.Gyanaec Surgeon from UK and Dr.Sergey Yokovenko from MOSCOW, graced the occasion along with dignitaries Dr.Manika Khanna, Director Gaudium IVF.

This conference intended by renowned gynecologist of Delhi served as an educational platform to discuss the existing problems such as PCOD and the scientific procedures which can serve as an effective cure to these problems. The usage of Letrozole formulation, which, was being given for treating infertility was discussed and deliberated at the session. Though there is statistically significant data to support letrozole for its advantage over CC (Clomiphene citrate) but it is true that letrozole is an acceptable alternative to CC as an ovulation induction agent in PCOS. Letrozole may be of benefit for patients in whom CC has failed.

In an elegant study by Tulandi et al. where in 911 newborns conceived after infertility treatment with letrozole and clomiphene citrate were accessed for congenital malformations, they found that there was no difference in the overall rates of major and minor congenital malformations among the 2 groups. However, it appeared that congenital cardiac anomaly was less frequent in the letrozole group. Till date there is no data to prove Letrozole is teratogenic (Substance that is able to disturb the growth and development of an EMBRYO or fetus)

Counselling before IVF can reduce stress

After completing the first cycle of Infertility treatment at a Bandra hospital two years ago, Nivedita (name changed) was advised 15 days of rest. The 28-year-old had been married for four years and had been unable to conceive. Owing to the stigma attached to Infertility, the Andheri-based couple did not tell their family that they were undergoing treatment.

Instead, Nivedita told her mother-in-law that she had a stomach infection. "It was so distressing to lie to my family and not share my problem with them," said Nivedita who feels that two subsequent failed cycles of in-vitro fertilisation (IVF) caused her a lot of stress.

Nivedita said some kind of psychological counselling would have helped her deal with it sit better. Most clinics ignore the psychological problems faced by patients. A survey conducted by Heal Foundation, a Delhi-based non-profit, found that 53% couples interviewed did not receive counselling before an IVF cycle. The survey, which was conducted in September, interviewed 50 couples each from 10 IVF centres in Delhi and Mumbai.

"The World Health Organisation recommends psychological counselling before a couple goes through IVF cycles. We feel it should be integral to the treatment," said Vikesh Khetrapal, principal consultant, HEAL Foundation. In Mumbai, some clinics have counsellors, but the majority do not address psychological problems faced by patients. Doctors claim  most patients are uncomfortable to discuss the matter. Also, it increases the cost of treatment.

But Nivedita found help with a Delhi-based Clinic run by Dr Manika Khanna that integrated psychological counselling with the treatment. "We had 10 joint counselling sessions with other couples. These helped change my perspective on infertility," she said. "Studies have shown that IVF success rate increases if psychological counselling is integrated in the IVF protocol. A patient's anxiety increases secretion of cortisol hormone. Cortisol inhibits production of estrogen and progesterone, which are needed for production of the egg," said Dr. Khanna.

Nivedita is now four months pregnant. "I attribute the success to the counselling sessions which gave me peace of mind."

With three failed in-vitro fertilisation (IVF) attempts behind them, the desperation of architects Prerna Shah, 38, and Vipul Patel, 39, (names changed on request) for their own child, has somewhat abated.

Although they still want to be parents, they don’t want to go through yet another IVF cycle.

“More than the time and money – we’ve spent R5 lakh over 5 years — IVF drains me physically and emotionally,” says Shah. “Each time I was given injections, I told myself this was it and I’ll finally go home with a baby. I didn’t happen. I see no hope,” she says. IVF involves daily injections, scans and invasive procedures for both partners. Additionally, many couples have to deal with repeated treatment failure and emotionally-tough treatment choices, such as whether or not to freeze embryos. The average success rate of IVF treatment in INDIA is 30% per attempt, with a woman’s chance of having a baby being 60-70% after three attempts.

Success rates are at a higher 35- 40% among couples below 35.

Only 44% IVF clinics in Delhi and Mumbai offer psychological counseling to couples, reported a two-city survey by the Delhi-based Heal Foundation. Sixty of the 100 couples surveyed said the psychological support offered was not sufficient. And though 72% doctors in Delhi and 81% in Mumbai knew how stressed clients were, they did not advise them to go for counseling. “Over 21 international studies support the fact that structured psychological-support counselling improves IVF success by 15-20%,” said Dr Manika Khanna, IVF expert and director, Gadium Clinic in Janakpuri. Dr. Manika Khanna, who does an average of 55 IVF cycles every month, offers a six session counselling module based on Boston IVF Centre’s modules to help under-treatment couples deal with stresses of Infertility and related complications.

“At the very onset, you need to assess the psychological strength of the patient, more so because success cannot be guaranteed,” says Dr Kaberi Banerjee, IVF consultant at Max Healthcare.
