A press conference was organised to mark the completion of two successful years and to announce the new initiatives undertaken in the domain of Infertility treatment. Chief guest – Professor Roy Homburg, Sr.Gyanaec Surgeon from UK and Dr.Sergey Yokovenko from MOSCOW, graced the occasion along with dignitaries Dr.Manika Khanna, Director Gaudium IVF.

This conference intended by renowned gynecologist of Delhi served as an educational platform to discuss the existing problems such as PCOD and the scientific procedures which can serve as an effective cure to these problems. The usage of Letrozole formulation, which, was being given for treating infertility was discussed and deliberated at the session. Though there is statistically significant data to support letrozole for its advantage over CC (Clomiphene citrate) but it is true that letrozole is an acceptable alternative to CC as an ovulation induction agent in PCOS. Letrozole may be of benefit for patients in whom CC has failed.

In an elegant study by Tulandi et al. where in 911 newborns conceived after infertility treatment with letrozole and clomiphene citrate were accessed for congenital malformations, they found that there was no difference in the overall rates of major and minor congenital malformations among the 2 groups. However, it appeared that congenital cardiac anomaly was less frequent in the letrozole group. Till date there is no data to prove Letrozole is teratogenic (Substance that is able to disturb the growth and development of an EMBRYO or fetus)

Counselling before IVF can reduce stress

After completing the first cycle of Infertility treatment at a Bandra hospital two years ago, Nivedita (name changed) was advised 15 days of rest. The 28-year-old had been married for four years and had been unable to conceive. Owing to the stigma attached to Infertility, the Andheri-based couple did not tell their family that they were undergoing treatment.

Instead, Nivedita told her mother-in-law that she had a stomach infection. "It was so distressing to lie to my family and not share my problem with them," said Nivedita who feels that two subsequent failed cycles of in-vitro fertilisation (IVF) caused her a lot of stress.

Nivedita said some kind of psychological counselling would have helped her deal with it sit better. Most clinics ignore the psychological problems faced by patients. A survey conducted by Heal Foundation, a Delhi-based non-profit, found that 53% couples interviewed did not receive counselling before an IVF cycle. The survey, which was conducted in September, interviewed 50 couples each from 10 IVF centres in Delhi and Mumbai.

"The World Health Organisation recommends psychological counselling before a couple goes through IVF cycles. We feel it should be integral to the treatment," said Vikesh Khetrapal, principal consultant, HEAL Foundation. In Mumbai, some clinics have counsellors, but the majority do not address psychological problems faced by patients. Doctors claim  most patients are uncomfortable to discuss the matter. Also, it increases the cost of treatment.

But Nivedita found help with a Delhi-based Clinic run by Dr Manika Khanna that integrated psychological counselling with the treatment. "We had 10 joint counselling sessions with other couples. These helped change my perspective on infertility," she said. "Studies have shown that IVF success rate increases if psychological counselling is integrated in the IVF protocol. A patient's anxiety increases secretion of cortisol hormone. Cortisol inhibits production of estrogen and progesterone, which are needed for production of the egg," said Dr. Khanna.

Nivedita is now four months pregnant. "I attribute the success to the counselling sessions which gave me peace of mind."

With three failed in-vitro fertilisation (IVF) attempts behind them, the desperation of architects Prerna Shah, 38, and Vipul Patel, 39, (names changed on request) for their own child, has somewhat abated.

Although they still want to be parents, they don’t want to go through yet another IVF cycle.

“More than the time and money – we’ve spent R5 lakh over 5 years — IVF drains me physically and emotionally,” says Shah. “Each time I was given injections, I told myself this was it and I’ll finally go home with a baby. I didn’t happen. I see no hope,” she says. IVF involves daily injections, scans and invasive procedures for both partners. Additionally, many couples have to deal with repeated treatment failure and emotionally-tough treatment choices, such as whether or not to freeze embryos. The average success rate of IVF treatment in INDIA is 30% per attempt, with a woman’s chance of having a baby being 60-70% after three attempts.

Success rates are at a higher 35- 40% among couples below 35.

Only 44% IVF clinics in Delhi and Mumbai offer psychological counseling to couples, reported a two-city survey by the Delhi-based Heal Foundation. Sixty of the 100 couples surveyed said the psychological support offered was not sufficient. And though 72% doctors in Delhi and 81% in Mumbai knew how stressed clients were, they did not advise them to go for counseling. “Over 21 international studies support the fact that structured psychological-support counselling improves IVF success by 15-20%,” said Dr Manika Khanna, IVF expert and director, Gadium Clinic in Janakpuri. Dr. Manika Khanna, who does an average of 55 IVF cycles every month, offers a six session counselling module based on Boston IVF Centre’s modules to help under-treatment couples deal with stresses of Infertility and related complications.

“At the very onset, you need to assess the psychological strength of the patient, more so because success cannot be guaranteed,” says Dr Kaberi Banerjee, IVF consultant at Max Healthcare.

Breastfeeding can be quite a challenge for working mothers. But there is a way to ensure the needs of both baby and mother are met.

“I was going through a dilemma when I was supposed to return to work after three months of maternity leave. How would I breastfeed her when I am not with her? My dilemma was: Should I get back to work or let go of my job,” asks Shilpa Sharma, an executive at a software firm and mother of a four-month-old baby.

Most working women face the same dilemma. Of course, breastfeeding is crucial as it is the ultimate source of nutrients for the baby. All doctors recommend that babies are fed exclusively on breast milk for four to six months. This ensures the baby's overall mental and physical wellbeing. But having to quit a job is not the solution.

Express and store

Most women do not know that breast milk can be expressed and stored in a ‘maternal milk container' before going to work, which is absolutely safe for the baby upto four hours at room temperature and a maximum of 72 hours if refrigerated. ‘Breast pumps' available in the market help mothers express more milk. Storing the milk in the maternal milk container helps keep its essential nutrients intact.

Once her dilemma was resolved, Shilpa got the best of both worlds. “Using a breast pump made a huge difference. Now I can leave the baby at home with my mother-in-law and still be sure she's getting the nutrients she needs.”

Returning to work does not mean the end of breastfeeding. For mothers who want to work after delivery means figuring out how to keep nursing without quitting job. Breast pumping gives them the freedom to leave the baby under someone else's care without having to worry about feeds.

It is advisable to start breast pumping and storing in a maternal milk container as soon as breastfeeding is established (when the baby is three or four weeks old) because many babies will refuse a bottle later. Every three or four days the baby should be fed the expressed milk.

Main challenge


The main challenge is to keep the nutrient value intact and keep the milk from spoiling. “Expressed breast milk cannot be stored in just any container as the quality of vital nutrients may get lost or contaminated, says Dr. Nomeeta Gupta, HOD, Department of Paediatrics, Batra Hospital, New Delhi. “The milk should be stored only in the maternal milk container and it should be kept in a cool environment soon after pumping.”

Breastfeeding is essential for overall well being and proper development of baby. But for working moms it can be quite a challenge especially if they work long hours or night shifts. A combination of ‘ breastfeeding and expressed milk' ensures that the babies get the essential antibodies via mother's milk to ward off infections and illnesses that can occur during the earliest years.

What are these? Breast Pumps are mechanical devices used to extract milk from the breast of the lactating mother. These are absolutely safe and do not have any long-term side effects. Rare feat in IVF history-Healthy twins delivered at the age of 50 using woman’s own eggs…
“It is rare but not impossible”, says IVF specialist Dr. Manika Khanna


Due to recent advancements in the field of Assisted Reproductive Technology PREGNANCY over age 50 has become possible using donor eggs. Recently a woman at the age of 51 delivered healthy twins using her own eggs at the Gaudium IVF & Gynae solutions. It is a breakthrough in the field of assisted reproductive (IVF) technology.

In the history of IVF treatment no case other than this has been reported where a 51 year old lady conceived and delivered healthy babies with her own eggs. She becomes the oldest first-time-in-vitro-mom on record.

The highest age reported for conception with IVF using own egg prior to this was reported from an IVF Clinic in New York where the patient was 49 yrs. The women in that case was 48 yrs old when her eggs were retrieved and frozen for one year and when she was 49 the eggs were transferred back to her.

Anita Mathur (name changed), 51 years old in her 23 long years of married life was unable to conceive. She took Infertility treatment at various hospitals and even underwent 2 failed IVF cycles at some prestigious IVF center in Delhi without any result.

She was shattered and heart broken and felt she will never be able to have a baby of her own. She had lost hope of conceiving as she was approaching 50.  But one day she along with her husband finally decided to give another shot of IVF cycle at The Gaudium after hearing a lot good about it.

Anita with her husband visited Gaudium and met Dr. Manika Khanna. Dr. Manika and team discussed her case and after carrying out her hormonal profile, they accordingly started her IVF cycle. To achieve the results Dr. Manika and team did lot priming with the hormones and to their utmost surprise they could help Anita in producing good eggs even at this age.

Dr. Manika revealed, “Over the reproductive years of women the quality and quantity of the eggs produced get diminished. By the time women reach 50 the ovaries are not able to release enough of the eggs and since the better eggs are used first, those latter produced are usually abnormal. It becomes a challenge when it comes to assist in conceiving and delivering healthy child using woman’s own eggs after 45.

After a month, IVF was performed on her using her own eggs (instead of donor eggs) and husband’s semen. Special care was taken for the procurement and the storage of eggs that resulted into the retrieval of good eggs. It resulted in her conceiving and delivering healthy twin babies after 9 months. The babies are now 9 months old and absolutely healthy.

Meticulous work by Dr. Manika Khanna with her team, specialized equipments at The Gaudium IVF and their extra care helped the couple to enjoy their parenthood.

Undeniably, this is a cause for celebration for the happy parents, for the world of reproductive technology, and for the millions of women who long to experience PREGNANCY and motherhood.

 

FACT : It is only partly true. Most of the patients either needs only medical advice or small procedures like IUI, laparoscopy, ultrasound monitoring, which are well within the reach of a common man.

However ART (assisted reproduction and technology) like Test Tube Baby and ICSI are costly. But only 10-15% of patients needs ARTs, Moreover, due to the globalization of world market, ART is becoming popular, acceptable and cost effective.

MYTH: IS INFERTILITY A WOMEN PROBLEM ?

 

FACT : Infertility is a medical problem.

Approximately 35% of Infertility is due to female factor and 35% is due to male factor. In the balance of cases, Infertility results from problems in both partners or the cause of the infertility cant be explained.

 

 

FACT : Removing sugars from the diet, (e.g., caffeine, white flour, white sugar, white corn, white rice, etc.), will improve ovulation and regularity. Foods that are broken down quickly raise insulin levels too quickly and disrupt delicate hormonal values in the ovaries causing more testosterone to be produced, impairing egg quality and thus possibly lowering the chance of conception. In some women, high insulin levels may cause irregular ovulation, irregular periods, or polycystic ovarian syndrome (PCOS).

 

 

 

FACT : In a study of more than 2,000 women, " lengthy Infertility treatment " was listed as one of the four key causes of sexual aversion. (The other three were childhood sex abuse, rape, and painful labor ad delivery.) Most couples report a decrease in the frequency of sexual relations after a diagnosis of Infertility. What was once a source of emotional intimacy often becomes " love by the calender, "and infertile couple say they feel a loss of privacy, sometimes even envisioning a doctor in the room during sexual intimacy. Both male and female infertility patients report a decrease in their level of sexual satisfaction, with the women also reporting that they feel less comfortable with their sexuality. More than one infertility counselor has told us, "I've never seen a couple going through fertility treatments who felt they had a great sex life."

MYTH: A WOMAN MUST HAVE AN ORGASM TO CONCEIVE.?

 

FACT : Approximately one in ten married women has never experienced an orgasm, and millions of these women have conceived. Additionally, many people believe that when a woman achieves climax-especially after the man does-fertility may be slightly increased due to enhanced sperm movement created by a small suction effect that's thought to pull sperm into the women's uterus. There's a certain logic behind this theory. Yet while studies have shown that such a "vacuum effect" exists, whether it actually brings about a higher PREGNANCY rate is as of yet unproven
